Join our team as a Community Support Specialist partnering with adults to support behavioral health and substance use recovery this role youll provide personalized community-based support meeting clients where they are: in their homes in the office and out in the community.

This position is based at Adapt of Missouri a subsidiary of Compass Health Network.
- Partner with clients and families using a motivational strengths-based approach to support progress toward recovery and resiliency goals.
- Provide individualized holistic services addressing all dimensions of wellness including physical mental and social health.
- Assist clients in developing and achieving goals outlined in their Wellness Plans including accessing needed services such as healthcare housing financial support and community resources.
- Offer education and skill-building to promote independence in daily living symptom management and recovery.
- Collaborate closely with integrated health team members and community partners to ensure coordinated culturally responsive care.
- Support clients through transitions such as hospitalization and discharge planning maintaining regular communication to promote continuity of care.
- Complete timely documentation and uphold service and quality standards in alignment with agency guidelines.
- Provide transportation and other direct supports as clinically appropriate. Driving is an essential function of this position.
- High School/GED required
- Associates* or Bachelors degree in a human service-related field preferred
- Qualified addiction professional;
- Bachelors degree in a human services field from a college or university included in the U.S. Department of Educations database of accredited schools at http:/ ;
- Any four-year degree or combination of higher education and qualifying experience;
- Four years of qualifying experience; or
- Associate of Applied Science in Behavioral Health Support degree as designated by the Department of Mental Health.*
- Qualifying experience must include delivery of service to individuals with mental illness substance use disorders or developmental disabilities.
- A valid drivers license and agency established minimum automobile coverage required
- Bring relevant experience working with adults or behavioral health populations
- Are comfortable traveling to homes and community locations
- Communicate with empathy adapt easily and enjoy being part of a collaborative care team
- Possess excellent time management and writing skills
- Enjoy meeting new people
- New to the social services field and eager to learn and gain experience
