Location: Norwich NY

Schedule: Part-time up to 29 hours per week

Pay: $17.50- $18.75

The Community Services Direct Support Professional is responsible for providing direct care through implementation of Habilitation Plans. Specific duties and responsibilities include but are not limited to:

• Provide supervision and all direct care required to maintain safety and comfort of the person receiving services
• Transport or accompany people receiving services on trips to community outings appointments or recreation activities for the purpose of community integration and exploration
• Development and planning of inclusive community activities
• Establish and maintain positive relationships with area businesses
• Assist people receiving services with completing daily or routine domestic or personal care tasks
• Foster and promote skills leading to independent living
• Assist with the development of Habilitation Plans and following these plans for training and assistance
• Provide training and assist people receiving services to develop maintain and/or improve desired individual goals living skills and personal needs as assigned or required
• Provide supervision to people supported during program hours and during community outings as to ensure their health and safety
• Complete required documentation in conformance with rules regulations policies and procedures and billing practices
  
  

• High School Graduate/GED with some knowledge of or experience working with developmentally disabled preferred.
• Possess a valid drivers license acceptable to the agencys insurance carrier
