Community Health Worker

Salary: $25.00 $30.00 per hour

Were hiring a Community Health Worker to provide support and care coordination to members enrolled in our Enhanced Care Management and Community Supports programs. Youll work directly with individuals facing homelessness chronic health conditions behavioral health challenges and other barriers to healthcare access. This role requires travel throughout the county for home visits and community outreach.

Requirement: You must live in or within a reasonable commute (under 30 minutes) to Santa Cruz County.

• Conduct community outreach and educate individuals about healthcare and social service programs
• Help eligible members enroll in managed health plans
• Manage an assigned caseload and coordinate referrals from health plans
• Navigate members through healthcare housing behavioral health and social service systems
• Conduct home and community visits to assess needs and provide health education
• Connect members with medical housing behavioral health and community resources
• Advocate for individuals facing complex physical behavioral or social challenges
• Use motivational interviewing to encourage positive behavior changes
• Collaborate with healthcare providers social workers and community organizations
• Maintain accurate and confidential documentation
• Report required program and operational data

• At least 2 years of professional experience in community health outreach care coordination or a related field
• Proven experience enrolling members into managed health plans
• Bachelors degree in Social Work Psychology Sociology or a related field (equivalent relevant experience considered)
• Community Health Worker certification (preferred but not required)
• Knowledge of healthcare systems medical terminology and community resources
• Familiarity with Medi-Cal Medicare CalAIM Enhanced Care Management or Community Supports (preferred)
• Experience supporting individuals experiencing homelessness behavioral health conditions or substance use disorders
• Strong communication cultural sensitivity and advocacy skills
• Ability to conduct home visits and travel locally throughout the county

Hours: MondayFriday regular business hours (occasional evening or weekend availability may be required)
