Compensation:

• $50000$55000/year

What Youll Do:

• Conduct community outreach and health screenings
• Support Medicaid members and individuals with asthma or chronic health conditions
• Complete health assessments and questionnaires
• Conduct home visits (approximately 8 per month; some virtual)
• Connect community members with resources and health-related services
• Assist with environmental health initiatives and supply coordination
• Present educational workshops and community programs
• Maintain accurate records across multiple systems

Requirements:

• Fluent in Spanish and English (Required)
• Must live in Staten Island NY or within a few mile radius.
• Passion for community health and helping underserved populations
• Ability to work independently and stay organized
• Comfortable with outreach home visits and public presentations
• Tech-savvy and able to learn multiple systems quickly
• Healthcare social services community outreach or customer-facing experience preferred
