Job Summary
The Community Health Worker - Tribal Practices provides culturally relevant education on prevention and the promotion of holistic wellness for Indigenous people residing in Maricopa County. Additionally this position is a liaison between the NATIVE HEALTH Community Health and Wellness Department Medical Department and community members to facilitate access to services to improve diabetes-related health outcomes. The CHW reports directly to the Indigenous Wellness Coordinator.

KNOWLEDGE SKILLS AND ABILITIES
- Facilitate group classes proficiently using a wide range of skills and techniques to help participants understand their common objectives and assist them to achieve them.
- Understand how social determinants of health impact the community and participants.
- Knowledge of Arizona Indigenous tribes practices of framing gardening and traditional foods.
- Knowledge of client confidentially and client rights according to Health Insurance Portability and Accountability Act (HIPPA).
- Ability to pass blood pressure and blood glucose testing competencies.
- Strong problem-solving skills and assertive in challenging situations.
- Highly resourceful team-player with ability to also be effective independently.
- Creative thinker who actively seeks opportunities and proposes solutions.
- Demonstrates ability to write clear and concise reports of service activities and other programmatic reports.
- Strong interpersonal skills including tact and flexibility to work effectively with supervisor staff and participants.
- Advanced computer skills included MS Office specifically EXCEL PowerPoint Outlook and Word.
- Develop creative ideas manage and deliver diabetes-related education via digital content using a range of techniques across channels including websites social media and Zoom platform.
- Culturally competent; able to work with diverse workforce and client popluaiton.
Qualifications :
QUALIFICATIONS/EXPERIENCE/REQUIREMENTS:
- A bachelors degree in a health-related field is required.
- Experience in community outreach or health education is a plus.
- Knowledge of Electronic Health Records (EHR) preferred Athenahealth a plus.
- Must be able to work irregular hours to include evenings and weekends as needed.
- Must possess two years of specialized experience working within the American Indian/Alaska Native Community.
- Must possess a valid and unrestricted Arizona drivers license (within 30 days of hire if applicant is from out-of-state) with no DUI/DWI or reckless driving convictions in the last five (5) years. No more than two at-fault accidents in the last three years. Must maintain a valid Arizona drivers license during employment. Must be insurable under NATIVE HEALTHs liability auto policy.
- Must pass a criminal background check and obtain a Class I Fingerprint Clearance Card within the initial ninety (90) calendar days of employment and retain a valid card while employed with NATIVE HEALTH.
- Must pass substance abuse testing upon employment and submit to random testing during employment.
- Must provide an updated immunization (IZ) record at the time of employment.
- Must have a current (within 12 months) Tuberculosis (TB) skin report upon employment and provide an annual update.
- Must obtain Basic Life Support (BLS) certification within ninety (90) calendar days of employment and maintain certification during employment.
