We are seeking a dedicated and compassionate Certified Nursing Assistant (CNA) specializing in Long Term Care (LTC) to join our dynamic healthcare this vital role you will provide exceptional patient-centered care to residents in a long-term care facility ensuring their comfort safety and dignity. Duties
- Assist residents with daily activities such as bathing dressing grooming and mobility using safe lifting practices and assistive devices like Hoyer lifts.
- Monitor and record vital signs including blood pressure pulse temperature and respiration rates accurately documenting all patient information within Electronic Health Record (EHR) systems.
- Support medication administration under supervision ensuring adherence to HIPAA regulations and maintaining patient confidentiality at all times.
- Provide specialized care for residents with dementia or Alzheimers disease implementing memory care techniques to promote comfort and orientation.
- Assist with medical procedures such as catheter care phlebotomy (blood draws) and aseptic techniques during treatments to ensure infection control standards are met.
- Collaborate with nursing staff to develop personalized care plans for residents with complex needs including behavioral health infectious disease management or post-acute recovery.
- Ensure a safe environment by practicing safe manual handling techniques during patient transfers and responding promptly to emergency situations such as Level I Trauma Center or ICU incidents.
Experience
- Prior experience working in long-term care facilities or nursing homes is highly preferred.
- Demonstrated knowledge of medical terminology vital signs monitoring and basic life support (BLS) certification is required.
- Experience working with individuals with disabilities including developmental disabilities or behavioral health conditions is advantageous.
- Familiarity with EMR/EHR systems like Cerner or eClinicalWorks enhances documentation accuracy and workflow efficiency.
- Background in geriatrics or senior care settings with exposure to dementia or memory care is desirable.
- Previous hands-on experience in hospital environments such as ICU emergency medicine or trauma centers will be considered an asset.
- Ability to perform heavy lifting safely while maintaining proper body mechanics is essential for patient transfer methods and manual handling tasks.

This position works in a hospital or long term care facility and is responsible for providing direct patient care through the performance of a variety of nursing support tasks in accordance with established methods and procedures. The position may work on
rotating shifts and in a variety of care units (e.g. SNF ICF medical surgical etc.). The position may also provide home care for patients.
