Lutheran Social Services of Wisconsin & Upper Michigan is seeking a compassionate and organized Care Coordinatorto join our CLTS Waiver Program team in Waukesha County!
The Childrens Long-Term Support (CLTS) Waiver Program is a federally funded Medicaid initiative that helps children with developmental physical or severe emotional disabilities receive services that allow them to thrive in home and community settings. As a Care Coordinator youll be the primary point of contact for families helping them access and navigate essential services.
Please note: This is a continuous posting in anticipation of future growth opportunities in the Waukesha County area.
What Youll Do
- Assess childrens functional abilities using approved tools
- Develop and implement individualized service plans with families and providers
- Facilitate team meetings and coordinate services based on family-centered goals
- Maintain accurate documentation and meet all regulatory timelines
- Collaborate with internal teams and external agencies to support families
- Testify in legal proceedings when required
- Communicate clearly with families providers and team members
- Participate in staff development training and supervision

Qualifications
- Bachelors degree in a human services field (e.g. Social Work Psychology Special Education Counseling etc.)
- Minimum 1 year of experience working with children with disabilities
- Bilingual fluency in Spanish is preferred but not required
- Proficient in computer systems and electronic health records
- Strong interpersonal and organizational skills
- Valid drivers license and reliable transportation (MVR check required)
Work Environment
- Community-based with daily travel required
- Moderate noise level; occasional exposure to outdoor conditions
- Flexibility to meet family needs including crisis response
