Job Summary

Please note all eligibility will be reviewed before an offer is made.This position will collaborate with other professionals to evaluate patients medical or physical condition and to assess client needs as part of the interdisciplinary team. They work collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plans of care for both individual patients and the medical community.
Essential Functions:
-Provides psychosocial assessments of patients and families with social emotional interpersonal and/or environmental issues. Formulates biopsychosocial assessment disposition and treatment plans.
-Collaborate with other professionals to evaluate patients medical or physical condition and to assess client needs as part of the interdisciplinary team.
-Monitor evaluate and record client progress according to measurable goals described in the treatment and care plan.
Qualifications
Qualifications:
- Masters Degree Social Work or Masters Degree Mental Health & Behavioral Medicine required
- Licensed Clinical Social Worker (LCSW) Licensed Independent Clinical Social Worker (LICSW) or Licensed Mental Health Counselor (LMHC) required
- 0-1 years of clinical experience in medical setting required.
- Pediatric experience strongly preferred.
- Bilingual (Spanish/English) strongly preferred.

- May need assistance and guidance in ensuring the patients needs safety measures and concerns are brought forward. Emerging knowledge of internal and external resources.
- Developing and demonstrating interpersonal collaboration in a medical setting- inpatient or outpatient.
- May need coaching/guidance in this area.
- May provide some assistance and support with onboarding for new team members.
- Aware of opportunities in department/service initiatives; participates in initiatives at an entity level. Uses data to measure progress.
- Demonstrate fundamental skills under supervision. May need assistance in promoting collaboration among healthcare team members other colleagues and the organization to support and enhance patient care.

Schedule and Work Model:
- Monday through Friday ET 8am-4:30pm or 8:30am-5pm.
- Must be flexible to support on-site needs at medical practices in Chelsea Revere and Charlestown (approximately 2-3x/week but may vary based on case load) and at patient homes or schools for meetings.
- 2x monthly team meetings at Assembly Row in Somerville MA office required (1st and 3rd Wednesdays of each month).
- Remote work requires stable secure quiet compliant work station.
