Job Title: Social Worker Masters

Shift: Saturdays & Sundays 8a - 4:30p

Join our compassionate team at Trinity Health Grand Rapids Hospital where your skills in clinical social work will make a critical impact during some of the most vulnerable moments in our patients this fast-paced environment you will conduct psychosocial assessments provide brief solution-focused counseling and develop collaborative treatment goals. Your work will help patients navigate crisis situations overcome barriers and access needed community resources to ensure safe and effective care transitions.

• Provide social work services during the 8:00 AM 4:30 PM shift in the inpatient hospital on the weekends only

• Conduct thorough psychosocial assessments for patients and families in crisis.

• Develop individualized treatment and discharge plans that address emotional social and practical needs.

• Deliver brief counseling and crisis intervention.

• Coordinate with community agencies and internal resources to support safe care transitions.

• Serve as a patient advocate and liaison between care teams and families.

• Document assessments plans interventions and outcomes in a timely and accurate manner.

• Collaborate with the interdisciplinary care team to support holistic patient care.

• Education: Masters Degree in Social Work or a related field from an accredited institution.

• Licensure: Eligible for licensure as a Limited Licensed Master Social Worker (LLMSW) or Licensed Master Social Worker (LMSW) by the Michigan Board of Social Work.

• Experience: Minimum of 2 years of social work experience in a healthcare setting; experience in emergency behavioral health crisis or acute hospital preferred.

• Exceptional crisis intervention and psychosocial assessment skills.

• Strong emotional resilience and sound decision-making under pressure.

• Compassionate communication and active listening.

• Collaborative team player who thrives in a dynamic high-acuity environment.

• Familiarity with local resources and referral processes for at-risk populations.

• Accurate timely clinical documentation and case tracking.
