Job Summary

The IHSS Clinical Program Manager is a qualified registered nurse responsible for overseeing and coordinating clinical activities for a designated group of patients and staff specific to the Colorado IHSS this role you will report to the Director of Nursing (DON).


Duties/Responsibilities:
  • Supervises and evaluates a group of qualified Attendants to deliver client-centered care

  • Provide supervisory support for clients in collaboration and as agreed upon by the client Authorized Representative and Licensed Medical Professional

  • Administers skill validation for Attendants and documents their competency in IHSS and basic client safety procedures

  • Ensures assessment and correction of skills validation factors for assigned staff members

  • Counsels Attendants and staff on complex cases and potentially dangerous situations

  • Collaborates with Clients Authorized Representatives and Attendants in the event of medical issues

  • Participate in activities related to the provision of professional services for assigned clients

  • Maintains compliance with agency policies and procedures laws regulations and professional standards of practice

  • Coordinates client care services for assigned clients with the Authorized Representative the Case Management Agency the Licensed Medical Professional the client and other Home Health Agencies to ensure services continue without interruption

  • Review the care plan Physician Attestation and any documentation provided by the Case Management Agency during the initial enrollment with any change in condition and upon request of the client/Authorized Representative or the Case Management Agency

  • Provides services in accordance with the plan of care

  • Assists DON with development/implementation and keeps records of criteria for admission to service case assignments and case management

  • Reports to the DON any changes that might affect the agencys operations

  • Performs coordination as assigned during emergencies and understands the Emergency Plan

  • Investigates reports and documents complaints and critical incidents according to regulatory timelines

  • Maintains confidentiality of agency employee and patient matters

  • Assists in orientating new staff prior to client interactions

  • Verifies Attendant compliance with care plan tasks

  • Submits documentation within required timeframes

  • Supports after-hours on-call duties related to client safety

  • Performs additional duties as assigned by the DON


Required Skills/Abilities/Knowledge:
  • Evidence of adequate health status to perform duties

  • Ability to learn absorb and apply professional training

  • Ability to organize work procedures assume responsibility and effectively supervise others

  • Demonstrates excellent problem- solving techniques and abilities

  • Demonstrates effective knowledge of the federal and state regulations governing home health agencies and IHSS providers with the ability to ensure compliance

  • Able to see and hear adequately to respond to auditory and visual requests which relate to the coordination of clinical activities

  • Able to speak in a clear concise voice in order to communicate adequately with staff physicians and patients

  • Able to effectively deal with change

  • Excellent computer skills required


Education/Experience/Licenses/Certifications:
  • Graduate of an approved school of nursing licensed in the state of practice; that has at least one to three (1-3) years of experience as a registered nurse in a state licensed home health agency or other health care facility

  • One year of home health and IHSS program experience (preferred)

Undergraduate degree (preferred)
