Job Summary
The Heart Failure Coordinator provides leadership and oversight of the heart failure program to ensure delivery of high-quality evidence-based patient care across the continuum. This role coordinates multidisciplinary care supports patient and family education monitors quality and regulatory metrics and leads initiatives to improve clinical outcomes and reduce hospital readmissions. The coordinator serves as a clinical resource for staff collaborates with physicians and care teams and drives program development performance improvement and accreditation readiness to achieve organizational goals related to heart failure care.
Schedule:
- Full-time 80 hours every two weeks St. Cloud Hospital
- Day 7am-4:30pm Monday-Friday
Pay and Benefits:
- Pay based on experience and degree
- Starting pay begins at $92539.20per year and increases with experience.
- Salary range: $92539.20-$138860.80 per year
- Salary range is based on a 1.0 FTE reduced FTE will result in a prorated offer rate

Education:
- Graduate from a nursing program
- BSN or MSN preferred
- BSN or higher required within 36 months of being hired
Experience:
- 5 years Minimum in nursing practice in ICU Cardiology or Emergency Department setting
Licenses and certifications:
- Registered Nurse (RN) Current licensure in the State of Minnesota
- Basic Life Support (BLS) Current American Heart Association (AHA) Healthcare Provider card required
Additional qualifications:
- Highly developed analytical critical thinking and organizational skills
- Strong verbal and written communication skills
- Basic computer skills
