The Clinical Pharmacy Specialist Pediatrics partners with interdisciplinary healthcare teams to deliver safe effective and evidence-based medication therapy for pediatric patients. This role serves as a trusted clinical resource for physicians nurses pharmacy staff and patients while supporting organizational goals advancing clinical excellence and upholding Mercys mission values and Service Standards.
- Review patient records to ensure appropriate safe and effective medication therapy
- Evaluate patient conditions to identify gaps in treatment and optimize care plans
- Develop individualized medication regimens that minimize risk and maximize therapeutic outcomes
- Monitor patient responses to therapy and adjust recommendations accordingly
- Provide consultation to prescribers nurses and patients regarding medication selection dosing administration and potential adverse effects
- Identify and resolve medication-related issues including interactions contraindications and formulary considerations
- Document patient care interventions medication plans and outcomes accurately
- Work collaboratively with physicians nurses and interdisciplinary teams to ensure optimal patient care
- Serve as a clinical pharmacy resource for pediatric care teams
- Maintain strong professional relationships across departments
- Participate in care planning discussions and clinical rounds
- Evaluate medication orders for clinical appropriateness and operational efficiency
- Participate in investigational drug protocols as applicable
- Support and contribute to Medication Use Evaluations (MUEs) quality assurance initiatives and adverse drug event reporting
- Assist in developing and validating staff competencies related to pharmacy and patient care practices
- Maintain advanced knowledge in pediatric pharmacotherapy and specialty practice areas
- Serve as an active preceptor for pharmacy students residents and trainees
- Deliver educational programs in-services and clinical presentations to healthcare staff
- Contribute to ongoing clinical education initiatives within the organization
- Ensure compliance with regulatory standards accreditation bodies (e.g. Joint Commission) and organizational policies
- Perform required inspections medication storage checks and documentation (e.g. expiration dates floor stock audits)
- Complete mandatory safety training competencies and continuing education requirements
- Exercise sound judgment in prioritizing workload and utilizing resources responsibly
- Demonstrate proficiency in pharmacy information systems (e.g. Epic) for order verification and clinical documentation
- Utilize drug information databases (e.g. Micromedex) to support evidence-based clinical decisions
- Exhibit professionalism accountability and strong communication aligned with Mercy Service Standards
- Collaborate effectively across shifts ensuring continuity of care
- Maintain a positive respectful and team-oriented work environment
- Adhere to dress code hygiene standards and safety protocols
- Perform effectively in a fast-paced environment with time-sensitive responsibilities
Education:
Bachelor of Science in Pharmacy (BS) or Doctor of Pharmacy (PharmD)Licensure:
Active pharmacist license in the state of practiceExperience:
Completion of PGY-1 residency or a minimum of 4 years of hospital pharmacy experience (pediatrics or specialty area preferred)Certifications:
Board of Pharmacy Specialties (BPS) certification eligibility required; specialty certification preferred
