Job Summary
This is a full-time position working a hybrid schedule of 4 days in person and 1 day remote. This position also rotates weekends and holidays.Ideal Candidate:
- Liaison experience preferred
- IRF/acute rehab experience preferred
- Active clinical license in MA required (RN PT OT SLP LCSW/LICSW)
Summary
Responsible for establishing and maintaining strong connections with community partners healthcare providers and relevant organizations by promoting the hospitals services building relationships and facilitating seamless patient transitions between the community and hospital settings.

Essential Functions
-Develop and implement outreach strategies to connect with local community organizations physician practices clinics senior living facilities and other healthcare providers.
-Build and maintain strong relationships with key stakeholders to promote the hospitals services and create referral pathways.
-Educate community partners and healthcare providers about the hospitals specialized services programs and resources available to patients.
-Collaborate with physicians discharge planners case managers and other healthcare professionals to facilitate patient referrals to the hospital.
-Conduct assessments to identify healthcare needs and gaps in the community.
-Organize and participate in community events health fairs seminars and workshops to promote the hospitals services and engage with the public.
-Collaborate with the hospitals marketing and public relations teams to develop targeted materials and messages for community outreach efforts.
Qualifications
Education
Bachelors Degree Healthcare Management required or Bachelors Degree Public Relations required or Bachelors Degree Related Field of Study required

Licensed Clinical Social Worker LCSW - State License
Licensed Independent Clinical Social Worker LICSW - State License
Occupational Therapist State License - Generic - HR Only preferred
Physical Therapist State License
Registered Nurse RN - State License
Speech-language Pathologist State License
Experience
Community Outreach Experience 2-3 years required and Healthcare/Managed Care Experience 1-2 years required
Knowledge Skills and Abilities
- Knowledge of local healthcare resources and community organizations.
- Familiarity with healthcare regulations privacy laws and ethical standards.
- Strong communication presentation and interpersonal skills.
- Ability to establish and maintain positive relationships with varied stakeholders.
- Excellent organizational and time management abilities to manage multiple initiatives simultaneously.
- Valid drivers license and willingness to travel within the community as needed.
