Do you Enjoy working with seniors supporting caregivers and helping things run smoothly Join Lifes At Home Care as our Client Care Coordinator. Youll help new clients get off to a great start and stay connected with families and caregivers to make sure care is going well.

This is a 40-hour-per-week role primarily based in our office with about 25% local fieldwork including travel. Field time may vary with new admissions and client needs.

What youll do

Coordinate admissions caregiver matching and in-person first-shift introductions.

Check in with clients and caregivers resolve concerns and follow through on solutions.

Make targeted quality visits and group nearby stops to reduce driving.

Support caregiver orientation and coaching with the RN and present caregiver recognition awards.

Keep WellSky records current and coordinate with the Branch Manager RN and scheduler.

What were looking for

Care coordination client service case management or caregiver supervision experience.

Social work or human services degree preferred; experienced CNAs with strong office and coordination skills welcome.

Professional communication sound judgment discretion and dependable follow-through.

Strong computer and organizational skills; valid drivers license reliable transportation acceptable driving record and ability to meet background screening requirements.
