Chha Home Health Hospice

Responsibilities:
- Provide personal care to patients in their homes including bathing dressing and grooming.
- Assist with mobility and transfers ensuring the safety and well-being of patients.
- Administer medication as prescribed by healthcare professionals.
- Monitor and record vital signs such as blood pressure and temperature.
- Assist with medical documentation and maintain accurate patient records.
- Provide companionship and emotional support to patients.
- Follow care plans and instructions provided by healthcare professionals.
- Communicate effectively with patients their families and healthcare team members.

Skills:
- Strong interpersonal and communication skills.
- Ability to follow instructions and work independently.
- Attention to detail and ability to observe changes in patient condition.
- Knowledge of proper hygiene practices and infection control measures.
