HIGHLIGHTS
- About: PacMed 65 Health Center is a primary care model (clinic within a clinic) serving our elderly patients (age 62 and up) we are starting with 2 clinicians and will need 2 MAs (standard MA workflow working to the top of their scope) to help support the program. Value-based care team model goal is to minimize hospital admits/emergency department utilization for our complex patients.
----- Candidates MUST demonstrate excellent verbal and written communication skills as this role requires frequent coordination with outside facilities and physicians regarding patient care and information.
----- The perfect candidate will have strong admin experience. Please call this experience out in the profile. If they do not have this experience they will be redirected.
----- Candidate MUST have all scheduling restrictions/limitations resolved prior to start (ie. childcare transportation etc). This role is full time 40hr/wk and the clinic needs someone who can show up on time and as scheduled to provide excellent patient care.
