Key Responsibilities

Day-to-day responsibilities of this position:

• Organizing and scheduling annual Individual Program Planning (IPP) for up to 70 individuals.
• Writing annual evaluations for the IPP.
• Teaching training and gathering new information on rights for people with disabilities.
• Participating as a team member on the Treatment Team.
• Coordinating legal and guardianship issues for assigned caseload.
• Individual Rights advocate. Train individuals and staff monitors for rights violations resolves individuals grievances attends Human Right meetings and coordinates other meetings.
• Protected Health Information (PHI). Trains employees and staff on PHI gets guardians to complete PHI consent forms.
• Admissions transfers and discharges. Attends admission meetings as assigned completes necessary evaluations coordinates transfers and discharges writes discharge summaries.
• Assist with funeral and end of life planning. Coordinating with the guardian and family at the time of death. Provide education and resources for staff.

Qualifications

Minimum or day-one qualifications:

• Is licensed as a Social Service Worker in the state of Utah
• Has one year or more of professional experience working with people with disabilities
• Demonstrates sensitivity tact and professionalism when interacting with others
• Skilled in professional facilitation and staff training
• Demonstrates the ability to analyze complex information and communicate it clearly in writing
• Demonstrates exceptional interpersonal abilities and a proactive approach to problem-solving

Preferred or trainable/on-the-job qualifications:

• Compose and produce reports documents and related material
• Use State or agency-specific software application program(s)
• Observe details at close range and at a distance
• Observe physical conditions behavior or surroundings for potential areas of concern or inappropriate activity
• Hear sounds that may indicate potential areas of concern or inappropriate activity

About the Role

Caseworker IIs are fully trained and exercise independent judgment and discretion while working under the supervision of a licensed professional in either assessing human service needs for children youth elderly adults developmentally and/or intellectually disabled individuals or providing substance abuse counseling to clients in a correctional facility or state institution. Employee recommends appropriate care or intervention documents client/inmate progress compiles case histories and prepares other related reports. Develops treatment plans and documents progress. May perform field investigations and determine actions or decisions governing a clients situation. May investigate abuse exploitation and/or neglect referrals and recommend appropriate actions affecting the clients well-being and social functioning. May coordinate provider services and ensure the quality of services for clients. May perform 24-hour on-call services under supervisors direction. May require licensure as a Social Service Worker. Generally at the Department of Corrections the best qualified individuals for this job will have obtained certification as a Certified Substance Use Disorder Counselor. Some employees within this title may work in a correctional environment with an inmate population.
