Job Summary
Position Title: Case ManagerDepartment: Case ManagementGrade Range/Job Status: Full Time/Non-Exempt - HourlyReporting Relationship: Lead Case Manager Supervisory Responsibilities: NA Typical Schedule: Primarily M-F Days. Must occasionally be willing and able to work flexible hours/days including evenings and weekends reflective of the dynamic schedule of the organization. Position SummaryThe Case Manager provides service coordination to individuals with various disabilities needs and backgrounds. The Case Manager is vital to helping people with disabilities and their families find their way through the social services system while promoting self-determination and a person-centered approach. Hammer & NER Case Management provides services contracted with Hennepin County for people receiving disability services through CADI BI and DD programs. Individuals the case manager supports may live with mental health needs physical health conditions brain injuries developmental disabilities or neurodevelopmental disabilities. Many individuals supported by Hammer & NER Case Managers have a combination of many needs and a variety of skills and preferences that make them unique and impact service planning. All employees are expected to center model and champion Hammer & NERs core values: Person-Centered Relational Opportunistic and Stewardship in order to provide people with intellectual and other disabilities the opportunity to live life to its Duties and ResponsibilitiesService CoordinationTake person-centered approach to identify the needs and preferences of individuals on your resources to meet the unique needs of each referrals for services are not regular contact with providers to ensure quality service payment authorization for service Plan Development and MaintenanceIdentify goals needs and preferences of the annual support plan that integrates all necessary policy plan as necessary throughout the health safety and independence and respect the right for individuals to take individuals in expressing their preferences and having their voice -person Home VisitsMaintain a minimum of two in-person visits annually with all individuals either in their home or other community be willing to drive throughout the greater Twin Cities and occasional travel throughout the state.5. DocumentationDocument case notes in electronic record case notes for all work throughout the include a detailed subjective narrative of all tasks precise time of each completed task to allow for necessary billing of integrity in all documentation Knowledge and Qualifications Bachelors Degree in human services related field required by in disability services working with mental health preferred for CADI case to demonstrate patience with individuals experiencing difficult have a reliable vehicle and adequate driving record as this position requires travel to in-person visits at client with Microsoft Office Products (Word Excel Outlook).Ability to maintain tracking systems for multiple tasks to meet varying written and verbal communication with clients and other to adapt to frequent process -motivation to complete tasks to ask ability and commitment to working with individuals and teams that are mixed across lines of difference such as race gender-identity sexual orientation religion ability age class and immigrant be willing and able to work flexible hours/days including occasional evenings and weekends reflective of the dynamic schedule of the job description assigns essential functions.
