Case Manager
Manage clients cases from intake to discharge Follow clients progress keep Clinical Director informed on any issues/problems with any clients. The Case Manager is charged with coordinating on-going and ancillary care for the client that may include but is not limited to legal appointments vocational issues insurance coordination external referrals for ancillary care. The Case Manager will document all of the client activities that s/he is responsible for in the client chart.
Duties and Responsibilities:
1. The Case Manager may maintain a caseload and be involved in client treatment processes:
a. Responsible for conducting group process.
b. Maintains treatment plans master problems list progress notes and all
other required client documents pertaining to treatment.

c. Maintains contact with clients family on a weekly basis discussing
progress/problems depending on clients release of information.

d. Assures proper client consents and that other treatment documents are

e. Maintains contact with client on a daily basis provides one-on-one
sessions with clients two to three times a week if necessary to coordinate services.

f. Responsible for charting one-on-one sessions groups and any and all

g. Meets with client on an individual basis when necessary.
h. Corresponds with clients probation officer and the courts if applicable.
Maintains contact with Clinical Director and Program Director regarding caseloads.
j. Establishes individual assessment and treatment program.
k. Insures clients attend and participate in all required groups and complete
all related assignments
2. Responsible for attending and participating in mandatory weekly staff
meetings to discuss client care and all aspects of administration.
m. Knowledge of policy and procedure manual relating to program protocol.
n. Other duties as assigned.
3. Case Manager performs and maintains professional abilities in the following
areas:

Qualifications:
- Must have at least two (2) years clean and sober if incumbent has disclosed they are in recovery.
- Drug and Alcohol counseling certificate: RAS CAS CADC CATS CDC CAADAC
NAADAC or any other license or credentials relating to the chemical dependency
field. - Minimum of two (2) years previous experience in the addiction field.
- Ability to prioritize and provide appropriate treatment protocols.
- Maintains current certification for this position:
a. CPR
b. First Aid
c. HIPAA/Confidentiality Training
