The ICMS Case Manager will work with clients to assess housing stability provide stabilization services and coordinate multiple services to meet each clients specific needs related to housing health mental health and Duties & Responsibilities:Work with a diverse and marginalized client caseload experiencing many barriers including homelessness complex trauma medical mental health and substance useEnsure that case management services to clients are reflective of the Whatever It Takes model which emphasizes customer choice harm reduction trauma-informed care and psycho-social rehabilitationEnsure that each client on caseload has an up-to-date and comprehensive biopsychosocial assessment and that this assessment is used in collaboration with the client to create individualized case management plans designed to improve quality of life and improved health outcomesCollaborate with each client to develop creative goal plans aimed at improving overall well-being and housing stability; review and update quarterly and upon completion of goalsDevelop and maintain a complete accurate and current client file with all required documents and data in agency records and electronic databasesDocument all client contacts in PIP format (Purpose Intervention Plan) and in accordance with Housing Department productivity expectationsCoordinate with DHS and community partners to ensure clients are connected to primary health care and insurance to reduce need for emergency health care servicesMaintain confidentiality of client files per HIPAA and all applicable guidelinesResponsible for coordinating appointments transportation and follow-up services for clients accessing primary health care mental health care recovery services and other community resourcesTransport clients in your vehicle to various agencies and healthcare providers in the greater Los Angeles area to increase community support services and community reintegrationDevelop effective trusting relationships with clients with a focus on facilitating independence and maintenance of improved physical and mental healthAssist in the screening assessment and enrollment of clients including orientation to program policies resources and goalsMaintain a current thorough knowledge of community resources and utilize them to provide comprehensive wrap-around services to clientsAssist clients with becoming involved in daily activities scheduling and attending appointments budgeting socialization health maintenance sense of community and progress in recoveryWork cooperatively and cohesively with other clients of the staff team including participation in weekly staff meetings and staff trainingsQualifications: Bachelors degree preferred; or a minimum two (2) years case management experience working with individuals experiencing homelessness preferably those living with mental illness and/or substance addictions or an equivalent combination of education and experienceValid CA Drivers license reliable car auto insurance and an acceptable driving recordAble to transport clients in your vehicleDemonstrated knowledge of case managementAble to work in a high tolerance Harm Reduction model with clients who have multiple barriersSkill in non-violent crisis interventionComputer literate; able to effectively use computerized database for client file management with basic skills in Microsoft Word Outlook and Excel programsDetail oriented with strong time management organizational written verbal interpersonal and computer skillsAble to obtain and maintain CPR/Frist Aid certificationPreferred Qualifications:Minimum 30 WPM typing speedBachelors DegreeWork Environment: Combination of field and office environmentRegularly required to sit stand bend; occasionally lift or carry up to 35 occasion walk or drive to different local sitesWill necessitate working in busy and loud environmentsWill be exposed to elements like cold heat dust noise and odorMay need to bend stoop twist and sit throughout the day
