Case Manager Registered Nurse | Pediatric Home Health

This position has a unique emphasis on Pediatric Home Health services with the goal of expanding pediatric care across much of CentraCares Home Health and Hospice service area. The Case Manager in this role will help support the growth of our pediatric population and play an integral role in the development of a Pediatric Palliative Care and Hospice Program.

As we work to expand our pediatric footprint this position requires flexibility to provide care throughout our full service area to care for both pediatric and adult patients. As our Pediatric Program continues to grow this Case Manager will serve as a key clinical resource for Pediatric Home Health Palliative Care and Hospice services for Pediatric patients.

Common responsibilities include comprehensive assessments wound care infusion therapy care coordination and patient and family education related to new diagnoses and treatment plans. Youll receive extensive onboarding ongoing education and dedicated RN office support while providing care in the field.

Schedule:

• Full-time 72 hours every two weeks(10 shifts per two weeks)
• Days Monday - Friday 8:00 am - 4:30 pm
• Every 4^thweekend working rotating days/evenings and two holidays per year
  

Pay and Benefits:

• Pay based on experience and degree
  • Associate/Diploma in nursing starting at $39.87 per hour
  • Bachelors in nursing starting at $41.25 per hour
  • Masters in nursing starting at $42.62 per hour
    • Pay range: $39.87 - $66.56 per hour
• Milage reimbursement and extra pay for working weekend shifts

Qualifications:

• Graduate from an accredited school of nursing required
• Current Minnesota Registered Nurse license required
• Current American Heart Association (AHA) BLS Healthcare Provider card required
• Reliable transportation required
• Valid Minnesota drivers license a driving record in good standing and insurance liability coverage meeting CentraCares standards required
• Minimum of one year of RN experience
  

Based out of our office in Marketplace Mall in Waite Park the Home Health department primarily takes place in the communities we serve in the homes of our patients. Our staff provides services not just for St. Cloud but our surrounding areas as well including Buffalo Alexandria Little Falls and more.
