Case Manager Inpatient Respiratory Unit Full Time

This position is accountable for assessing planning implementing coordinating monitoring and evaluating options and services available to meet a patients health needs. Coordinates services provided to a specified group of patients ensuring that the care is provided in an appropriate setting and that the severity of illness and the intensity of service is consistent with the patients clinical condition. Collaborates with the health care team members to ensure that the care provided meets desired patient outcomes with appropriate utilization of resources within expected time-frames across the continuum of care. This position practices within the scope of the Nursing Practice Act and in accordance with the policies on Valley Childrens Healthcare.

Qualifications:

This position handles cases for the inpatient respiratory unit also known as Apollo.

Education

• College Degree - 2 year Nursing (required)
• College Degree - 4 year Nursing or in a field that supports the Case Managements process (preferred)

Licenses and Certifications

• RN - Registered Nurse License - CA-BRN - California Board of Registered Nursing (required)

Work Experience

• Minimum two (2) years Nursing experience (required)
• Pediatric experience and Case Management/Utilization Management experience desirable. (preferred)

Skills and Abilities

• Bilingual Skills Preferred.
