Position Summary:
The TAY HWCCase Manageroutreaches to intakes assesses counsels and coordinates care for TAY ages 18-27 years old to achieve safety health and well-being across multiple life domains such as housing/shelter education and employment behavioral health physical health community building family and relationshipsand much more. The Case Manager serves as the clients main advocate and care coordinator within GLIDEs continuum of care community partners and resources and other external systems of care.This position works closely and collaboratively with other TAY HWC team members toenable warm hand-offs and linkages amongst specialists case managers navigators and ambassadors.Uponexit fromGLIDEs care the Case Manager develops a care transition plan to ensure the clients are linked toadditionalsupport services and knows how to reconnect with GLIDE for future support.In addition to 1:1case management support this position will alsofacilitateand/or coordinate weekly activities or psychoeducation groups that will support promote and enhance clients confidence wellness community connection and growth mindset. Work schedules may include the need towork onone day during the weekend.Working hours may alsovary and occur during nontraditional hours.
Essential Duties and Responsibilities
- Initiate and conduct outreach with TAY at the TAY HWC Center and at offsite locations where TAY may access or congregate for population engagement.
- Administer intake and assessmentsto enroll TAY into GLIDEs continuum of careestablishbaseline ofsafety and health needsanddevelopindividual goal/service plans that include actionable steps for care coordination.
- Develop a care coordination plan based on 1:1sessionswith client referral information and/or assessment andidentify key care coordination needs within GLIDEs integrated services (e.g. Health Access Family Resource CenterMeals etc.)including GLIDE partners(e.g. UCSF Street Medicine Clean Slate Larkin St.) and establish successful linkages to care.
- Support TAYto navigate and overcome barriers relatedsafetyhealth and economic stabilityby teachingand coachingTAYon how to self-advocatecomplex service systems using effective communication tools and strategies.
- Identify personal knowledge and skills gaps that may negatively affect TAY ongoing stability and success andlink TAY to educational resources to increase essentialknowledge and skillsincluding but not limited to sexual health educationpersonal hygiene pet careharm reduction principles building healthy relationships adult literacy digital literacy interfacing with banking institutions etc.
- Coordinatefacilitate and/or directly conductexpressive arts activitiestherapeuticactivities or psychoeducation groups that will support promote and enhance clients confidence wellness community connection and growth mindset.
- Monitor client progressthrough regular scheduled appointments and follow-upto ensure successful implementation ofindividual service planandmaintainaccuratetimelyand up-to-date records of TAY intake assessments progress and outcomes in required databases and hard copy files asrequired.
- Participate in regular staff meetings staff training events supervisory meetingsand community meetings to build andmaintainstrong relationships withother GLIDE programs community partners and City agencies to enhance service delivery and client support.
Minimum Qualifications
- Bachelors degree in social work public health community healthor related fieldpreferred.
- 5years of experience inprovidingintensive case management to TAY and/or similarlydisenfranchisedand high need populations in social service settings.
- Experience anddemonstratedsuccess providing 1:1 counseling case management.
- Knowledge of Bay Area San Francisco TAY needs and resources especially those experiencing poverty homelessness mental health challenges substance misuse trauma and other associated experiencesand how to navigate and link TAY to those resources successfully.
- Must have experiencefacilitatingthe followingactivities andworkshopssuch asweekly activities or psychoeducation groups thatsupportsandenhancesclients confidencewellnessandcommunity connection.
- Ability to work independentlydemonstrate initiative be dependable and have consistent presence.
- Ability to collaborate withinternal and external stakeholdersto coordinate care for shared clients to help clients stabilize and reach their individualgoals.
- Strong written and oral communications skills including excellent interpersonalskills.
- Attention to detail strong organizational time management and problem-solving skills.
- Ability to use computer and reporting systems including Windows operating system Microsoft Office Suite Salesforce or other database systems Zoom (or other virtual meeting systems) and search engines for research.

$28.84 - $33.65 an hour
This is a part-time (not to exceed 16 hours/week) non-exempt role.
Work Environment
GLIDEs buildings are located in the Tenderloin maintains an open door policy to the community and its clients who frequently enter and leave the building.
This role is covered by the collective bargaining agreement.
