With a staff of 950 and an annual budget of $140 million Project Renewal is one of the larger social service nonprofits in New York : Harm Reduction Case Manager Program: In Homes NowSalary Range: $44000 - $48000 Program Description:In Homes Now provides 116 formerly homeless men and women with permanent housing. The multi-disciplinary team works together to ensure that each tenant successfully remain in their housing. The approach used in this program is a Harm Reduction/Housing First Model. Housing is provided through independent apartments rented by Project Renewal throughout the Bronx Brooklyn and Manhattan. The case management program provides assessment treatment planning advocacy and referral Duties and Responsibilities:The essential duties of the Case Managerinclude but are not limited to the following activities:Case Manager for a caseload of 16 to 18 individualsMeet with clients a minimum of 2 times per month in the homeAssist clients with referrals for medical/dental and substance abuse servicesMaintain accurate and up-to-date clinical and statistical documentationComplete service plans every 6 monthsComplete psychosocials annuallyEscort client to appointmentsAttend and participate in all staff and team meetings and in-service training; case consultation with interdisciplinary staffQualifications:Bachelors degree preferredAbility to complete assessments and service plansExperience working with individuals who are homeless and/or those affected by substance abuseStrong time management and writing skillsSpanish fluency a plusProject Renewal is an equal opportunity employer.
