With a staff of 950 and an annual budget of $140 million Project Renewal is one of the larger social service nonprofits in New York : Case Manager Program: Atlas House Mens Shelter Salary Range: $45000 - $48000Program Description:Atlas House Mens Shelter is a 110 bed 24/7 general population emergency shelter for homeless men in the Flatbush area of Brooklyn. Our comprehensive services include case management individual and group counseling recreational activities and housing placement assistance. Additionally on-site medical and psychiatric services are offered in addition to referrals to a wide variety of resources and services that our clients may Overview: Under the general direction of the Director of Social Services the Case Manager is responsible for providing the required assessment case management and counseling services to a designated caseload of clients. The Case Manager works collaboratively with each client to assist them in identifying and securing community-based housing as rapidly as Duties and Responsibilities:The essential duties of the Case Manager include but are not limited to the following activities: Provides case management to a designated caseload of intake psychosocial and housing assessments of collaboration with each client prepares and monitors an Independent Living Plan identifying short- and long-term objectives to be achieved. Inputs all client information and updates into the CARES system. Counsel clients through individualized and group sessions on issues such as well-being self management and independent living skills. Identifies and makes referrals for community-based treatment and support clients authorization coordinates care with community-based collaboratively with the Housing Specialist to identify and reinforce each clients housing clients for housing with on-site psychiatric provider nursing and primary care to ensure each client has an appropriate aftercare :All candidates must have at least a Bachelors degree from an accredited college or university.A minimum of three (3) years of experience working with mentally ill individuals or homeless individuals with either a diagnosis of mental illness or a substance use disorder is Renewal is an equal opportunity employer.
