Job Summary

Meets the needs of clients by providing case management and rehabilitation services to facilitate positiveoutcomes. Works with children youth and their families to secure support services entitlements medicalcare community resources and family support systems that enhance emotional wellbeing and supportsuccess at home in school and in the community.


Hours:40 hours per week; Monday - Friday 8:00 am to 4:30 pm

As a Case Manager you will:

  • Promote healthy development and positive outcomes for families through utilizing case management and rehabilitative strategies with a team of dedicated professionals
  • Assist clients and families to build upon natural support systems and links individuals to other service agencies.
  • Provide additional supportive services to assist with continuity of treatment linkage to resources transition from program to program and transition from program to community resources
  • Connect every client/family to a medical home as well as clinic school home and community-based services
  • Serve as an advocate for the client and family
  • Create and maintain all required documents in the electronic and paper record systems

Our Children Youth and Family (CYF) outpatient programs (Crossroads Family CenterDouglas Young Youth and Family Services Mobile Adolescent Services Team (MAST) and Nueva Vista Family Services provide services throughout San Diego County. We provide a range of services including crisis intervention individual family and group therapy medication support rehabilitation case management and drug and alcohol services while incorporating the family systems and wrap-around approach.

We provide mental health services to children and youth with Medi-Cal as well as those who are uninsured and underinsured. Services are provided in English and several threshold languages using medical care education and family resources as appropriate.

CRF staff members work together to meet the needs of San Diego Countys residents who are experiencing mental/behavioral health challenges. Education:High School/GED diploma with at least one year of experience working with children and adolescents with severe emotional disturbance in a public mental health setting required. Bachelors degree in a mental health related field such as Psychology Social Work or Counseling with child and adolescent experience preferred.

Additional Qualifications and Skills

  • Bilingual Spanish is required for this position
  • Experience working with children and adolescents with severe emotional disturbance in a public mental health setting
  • Ability to make assessments design treatment plans and document accordingly
  • Ability to organize and prioritize multiple concurrent assignments and meet productivity goals
  • This program is housed in a two-story building that does have an elevator; however ability to navigate stairs may be required.
  • Ability to pass criminal/background clearance
