Job Summary
Remote Case Management Pharmacist Coordinate Care for Complex Patients Work From Home
Support high-risk and complex patients through medication reviews care coordination and adherence support all from a remote setting. Ideal for pharmacists who enjoy clinical problem-solving and want a more sustainable schedule.
Key Responsibilities
- Collaborate with physicians nurses and care coordinators to manage high-risk or complex patients.
- Conduct medication reviews to identify gaps in therapy adherence concerns or potential interactions.
- Support prior-authorization and appeals processes when needed.
- Educate patients and caregivers on regimens and disease-state management.
- Document case activities and outcomes in compliance with health-plan and regulatory standards.
- Participate in quality initiatives aimed at reducing hospitalizations and improving outcomes.
What Youll Bring
- Education: PharmD or Bachelor of Pharmacy.
- Licensure: Active U.S. pharmacist license.
- Experience: Case management MTM or managed care preferred. Hospital ambulatory and community pharmacists with strong counseling backgrounds are encouraged to apply.
- Skills: Strong communication problem-solving and care-coordination abilities.
