Location: Field Based - Arlington Alexandria Annandale Springfield Falls Church

Travel: Up to 75%

Position Summary
CaseManager Analystisresponsibleforfacetofaceandtelephonically assessingplanningimplementingandcoordinatingallcase

management activitieswithmemberstoevaluatethemedicalneedsofthememberto actiontoaddressissuespresentedtoenhancetheshortandlong- term outcomesaswellasopportunitiestoenhanceamembersoverallwellness comprisedofnetworkmanagementandclinicalcoveragepolicies.

Required Qualifications

• Must reside in Annandale Falls Church Springfield Arlington Alexandria VA area
• Must possess reliable transportation and be willing and able to travel up to 75% of the time. Mileage is reimbursed per our company expense reimbursement policy
• 2 or more years experience in behavioral health social services or appropriate related field

Preferred Qualifications

• Case Management Discharge Planning and/or Managed Care Experience

Education

• Bachelors Degree or non-licensed master level clinician required with either degree being in behavioral health or human services preferred (psychology social work marriage and family therapy counseling) LMHP RN/LPN QMHP LMSW LBSW MSW or BSW.

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience education geography and other relevant factors. This position is eligible for a CVS Health bonus commission or short-term incentive program in addition to the base pay range listed above.
