Starting Pay:$16.25/hourAdditional $0.50/hour for 14 years of experienceAdditional $1.00/hour for 5 years of experienceAdditional pay for education in a related fieldPreliminary Requirements:Proof of non-probationary drivers license held for at least one yearAbility to attend orientation (TuesdayFriday Daytime)The Case Coordinator is responsible for providing organizational assistance and programmatic support this includes admissions procedures direct participant care and internal/external coordination of JOB FUNCTIONSTo perform this job successfully an individual must be able to satisfactorily perform each essential function listed below: Develops and assures that the program meets the participants needs optimizing the participants potential to be integrated into community. Ensures the provisions of appropriate services to designated program participants. Assists designated program participants as needed to understand the overall goals and programs being implemented in the process as requested. Acts as a liaison for families along with staff and provides them with information and resources as requested. Organizes and chairs program participant conferences. Reviews the designated program participants reports (evaluation progress and discharge) for consistency and coordination of goals. Ensures the appropriate documentation/record keeping of designated program for development coordination carry-through of and follow-up on discharge planning as directed. Assists in internal communication to team members relative to case-specific issues. Attends required meetings including clinical team meetings and regularly scheduled supervision meetings. Reviews monthly bills for designated program participants after receipt from accounting staff. Identifies any discrepancies to accounting staff. Coordinates utilization with the Case Manager relative to program participants incentive account Performs other related duties and activities as job functions for NeuroRestorative NJ Program: Organizes and facilitates internal medical appointments. Ensures the appropriate documentation/record keeping of designated program participants for internal medical appointments and distribute timely/appropriately. Prepares summaries of internal medical appointments and as requested tracking information. Oversees the annual medical appointments are completed timely Responsible for the development coordination carry-through of and follow-up on healthcare planning as directed by the Program Director/Manager. Reviews the designated program participants reports (evaluation progress and discharge) for consistency and coordination of healthcare needs. Ensures appropriate medical follow up to designated program participants. Reviews with and assists the designated program participant as needed to understand overall healthcare needs as requested. Communicates with community resources and persons outside of the program to assure quality program participant healthcare for all assigned program participants. Assists in training new staff involved with overseeing medical careSUPERVISORY RESPONSIBILITIES None Minimum Knowledge and Skills required by the Job The requirements listed below are representative of the knowledge skill and/or abilities required to perform the job: Education and Experience: High School Degree required an Associate or Bachelors Degree in Social Service related field preferred. Two years of direct or indirect support services to the disabled population in a medical and/or rehabilitation setting. Certificates Licenses and Registrations: None requiredOther Skills and Abilities: N/A Other Requirements: Travel as neededPhysical Requirements: Light work. Exerting up to 20 pounds of force occasionally and/or up to 10 pounds of force frequently and/or a negligible amount of force constantly to move objects. If the use of arm and/or leg controls requires exertion of forces greater than that for sedentary work and the worker sits most of the time the job is rated for light work.
