Key Responsibilities:
- Perform member outreach telephonically or through home visits to engage members and discuss care/service plans.
- Coordinate care activities based on the care plan/service plan and collaborate with healthcare and community providers.
- Serve as support for various member and/or provider inquiries related to care plans.
- Document and maintain member records in accordance with state and regulatory requirements.
- Identify needs and make referrals to Care Managers community-based organizations and Disease Managers.
- Provide education on benefits and resources available to members.
- Comply with all policies and standards while performing other duties as assigned.
- High School diploma or GED.
- 12 years of relevant experience in care coordination managed care health plan member services patient support or case management.
- Strong communication attention to detail multitasking and documentation skills are essential.
