Blended Case Manager Nanticoke

Northeast Counseling Services (NCS) is seeking a compassionate motivated and dedicated individual to join our team as a Blended Case Manager. This position provides services in community settings rather than an office environment allowing you to directly support individuals where they live work and participate in their communities. Flexible schedule is available to help promote work-life balance.

The Blended Case Manager works with individuals experiencing serious mental illness using a strengths-based recovery-oriented approach. The primary goal of this position is to empower clients to identify their needs access resources and achieve greater independence through coordinated support and advocacy.

Must maintain a valid PA Drivers license.

High School Diploma or equivalent is required.

Job Type: Full-time

Pay: $31200.00 per year to $33000.00 per year and up based on education and experience

Education:

High School Diploma or GED.

• Minimum of 12 college credit hours in Sociology Human Services Psychology or another social science field is required.
• Two (2) years of experience in Human Services with 1 year of providing direct client contact.
• Bachelors Degree in Social Work Psychology Human Services Sociology or a related field preferred

License/Certification:

• Drivers License (Required)

This position requires the successful completion of an FBI Clearance PA Criminal Clearance and Child Abuse Clearance.
