As a trusted leader youll have the unique opportunity to enhance patient outcomes while mentoring a dynamic team. Heres how youll make a difference:

• Lead Exceptional Care Delivery: Oversee and coordinate home health services tailored to behavioral health needs. Manage patient and provider assignments ensure compliance with regulations and prioritize timely starts of care.

• Champion Quality & Compliance: Approve and evaluate individualized care plans to maintain the highest clinical standards. Seamlessly integrate and coordinate physician orders.

• Build a Culture of Compassion: Inspire and support a team of healthcare providers ensuring patient needs are continuously assessed and met with excellence.

• Streamline Operations: Manage processes for physician orders staff communication and interdisciplinary coordination for holistic care.

• Education: Associates Degree in Nursing (Bachelors preferred).

• Experience: At least 2 years as a Registered Nurse in Home Health or Behavioral Health with 1 year in a leadership role.

• Expertise: Familiarity with enterprise systems (Workday HomeCare HomeBase) and MS Office Suite.

• Licensure: Current state RN license.
