Job Summary
Overview:We are making dreams come true. Our organization provides children and adults with autism intellectual disabilities and behavioral health challengesand their familiesthe education support and resources needed to be self-reliant independent and valued members of their are seeking a dedicated and person-centered Program Specialist to coordinate services and supports for individuals participating in our Adult Day programs. This role serves as a key member of the interdisciplinary team ensuring Individual Support Plans (ISPs) are implemented effectively regulatory requirements are met and individuals receive high-quality services that support independence choice and community Youll Bring:A passion for supporting individuals with intellectual and developmental organizational and time management communication and relationship-building to collaborate effectively with families staff support coordinators and community problem-solving and critical-thinking to person-centered planning and community to train and support staff on individual-specific needs and regulatory to detail and a commitment to documentation accuracy and Youll Have:One of the following qualifications:Masters degree and one year of experience working directly with persons with disabilities; orBachelors degree and two years of experience working directly with persons with disabilities; orAssociate degree 60 college credits or completion of a two-year program and four years of experience working directly with persons with unlicensed programs five years of experience in the human services field may be substituted for educational with electronic health record systems such as CareLogic record management and documentation drivers license to complete CESP or ACRE-approved Basic Employment Services training within nine months of hire if 33 and Act 34 clearances required; FBI clearance required if applicable.A Typical Day May Include:Coordinating services and supports for assigned individuals and ensuring ISP goals are implemented in ISP meetings assessments quarterly reviews admissions and discharge documentation for compliance accuracy quality and progress toward individual goals and ensuring services align with personal interests and and supporting staff on ISP implementation behavior plans and individual-specific regulatory due dates and ensuring required documentation is completed on and updating electronic records within health safety rights and well-being of individuals receiving with Operations Supervisors families support coordinators and other service with incident reporting investigations and follow-up activities as and analyzing program data and preparing reports to support program community integration employment initiatives and meaningful day programming in committees quality improvement activities and annual training requirements.
