GENERAL PURPOSE
The Aging Care Management Supervisor 1 Program Support position is responsible for the direct supervision of a unit of Care Managers which works within the Countys Area Agency on Aging to provide services to the elderly. This role may also supervise the provision of direct services in any one of several programs within the Countys Area Agency on Aging.
SUPERVISION RECEIVED
This position reports directly to the Area Agency on Aging Administrator 3.
SUPERVISION EXERCISED
This position is responsible for the supervision of a unit of Aging Care Managers.
ESSENTIAL DUTIES OF THE POSITION
Oversees the quality of services rendered by unit personnel at care plan development service delivery and monitoring phases through unit meetings individual conferences and case/record reviews as well as documentation maintained in the data system (SAMS) and hard copy/electronic files for OPTIONS and Caregiver Support Programs.
Monitors for required forms and journal entries.
Assists staff via continuing consultation on cases as well as for discussions of concerns relating to advocacy and the appropriate utilization of agency and community resources.
Orients and arranges for training for PA MEDI (health insurance counseling) volunteers assists with on-boarding of volunteers and monitors on-boarding process.
Organizes and supervises volunteers work and establishes specific day and time for regular meetings. Prepares materials and sends out notices and information.
Plans flexible agendas for each meeting to ensure volunteers make most effective use of time. Consults with volunteers on program policy/procedure and consumer concerns.
Counsels consumers on health insurance and long-term care coverage needs.
Coordinates needs and outreach efforts of volunteers/counseling sites.
Approves time sheets and manages leave requests.
Assigns cases and supervises work flow.
Develops written procedures guidelines and internal policies for unit operation.
Gathers processes and disseminates information from Agency administrative and unit staff. Represents unit concerns at the supervisory and administrative level.
Approves time certifications and expense reports.
Completes Performance Evaluation Reports.
Administers discipline as necessary.
Functions as back up to care managers and provides direct service in the absence of the worker. May also function as a back up to Protective Services Supervisor as needed.
Assists staff in the performance of their duties and in select situations intervenes with consumers and the community.
With the Administrator interviews candidates for employment in the OPTIONS and Caregiver Support Units.
Coordinates unit functions and procedures with other Aging units.
Contacts other agencies and segments of the community and interprets Agency programs and service. Assists in planning to meet community needs.
Oversees the training of new staff and interns assigned to the Agency for field placement.
Schedules develops and presents in-service training for staff as needed.
Maintains confidentiality at all times.
Must be available to cover on-call hours when required.

REQUIRED MINIMUM QUALIFICATIONS
EDUCATION & EXPERIENCE Graduation from an accredited college or university with a Bachelors degree;AND
At least three (3) years of full-time professional experience in public or private social work. (Public or private social work is defined as professional work identifying and evaluating consumers problems and formulating implementing or directing an appropriate service place. This requires in-depth interviewing to obtain comprehensive information about consumers needs; counseling; providing information about available programs; determining eligibility for program services; and negotiating on behalf of consumers with referral agencies to successfully implement appropriate services.);OR
At least three (3) years of full-time professional experience in public or private social work (as defined above);AND
The successful completion of twelve (12) semester hours of college level courses in Sociology Social Welfare Psychology Gerontology or other related social sciences.
NOTE: Graduate coursework in the behavioral sciences may be substituted on a year for year basis for up to two (2) years of the required experience. Behavioral sciences include but are not limited to Anthropology Counseling Criminology Gerontology Human Behavior Psychology Social Work/Social Welfare Sociology and Special Education.
Employee assigned to this title will be required to possess and maintain a valid and current motor vehicle operators license. Must have an acceptable driving record. Personal vehicle required for County business; employee is eligible for mileage reimbursement.
KNOWLEDGE SKILLS AND ABILITIES
Knowledge of social casework principles and methods.
Knowledge of current social economic and health problems and community resources as related to older adults.
Knowledge of individual and group behavior and ways of working effectively with older adults who have social economic emotional and/or health problems.
Knowledge of the basic principles and methods of program interpretation and community organization.
Skill in the development of plans to address the needs of older adults.
Ability to work effectively with older adults and aid them to grow via the constructive use of their potential and adjusting to their specific problems.
Ability to evaluate work of subordinates and to provide technical assistance and guidance.
Ability to orient and instruct subordinates on procedures techniques laws and regulations.
Ability to establish and maintain effective working relationships with clients other members of the staff outside agencies and institutions and the general public.
Ability to clearly express ideas verbally and in writing.
Ability to maintain confidentiality at all times.
Ability to read write speak understand and communicate in English to perform the duties of this position.

While performing the duties of this job the employee is frequently required to walk sit talk and hear.
