This opportunity is located on the north side of Indianapolis at Community Hospital North and Community Heart & Vascular Hospital.

Highlights
Community Physician Network is seeking a Nurse Practitioner or Physician Assistant to work alongside our Pulmonary Hypertension team at Community Hospital North in Indianapolis IN.

• 4-10 hour shifts weekly including in office and virtual home visits. Hours may vary based on patient volume/needs
• Opportunity to practice in the Pulmonary Hypertension clinic and inpatient hospital rounding.

Qualifications

• Experience practicing as a licensed Nurse Practitioner or Physician Assistant isrequired.
• Pulmonary Hypertension Heart Failure or Palliative Care Experience is strongly preferred.
• Must have excellent communication critical thinking and planning skills
• Must be hard working proactive and dedicated to delivering high-quality patient care
• Must be comfortable assessing patients independently

Licensure & Certifications
Educational background consistent with requirements for licensure and certification within the state of Indiana
