The Activities Coordinator oversees our Oshkosh and Green Bay Day Service programming. They will work as an active team member providing support to participants and staff. They will also lead the implementation of Chance events and assists with the execution of program objectives. They carry out responsibilities in the following functional areas: HCBS compliance planning risk management facilitation and customer service.
- Ensure that Clarity Care programs operate in compliance with laws regulations managed care organization contractual guidelines and organizational policy.
- Create and implement effective oversight systems for all programs supervised. Evaluate quality metrics and outcomes achieved by Clarity Care on a monthly basis and undertake corrective actions where findings dictate.
- Assess and evaluate staff development needs and ensure that all such needs are adequately and routinely addressed through utilization of internal and external training and education resources.
- Monitor physical plant and fire safety inspections and ensure all safety hazards are immediately addressed.
- Supervise the financial management affairs of the Day Service programming in a prudent manner with attention to fiscal forecasting standard accounting principles sound record keeping and integrity of consumer accounts and residence funds.
- Monitor vacancy and occupancy levels to achieve goals.
- Ensure full compliance with the Wisconsin Department of Health Services Home and Community Based Services (HCBS) settings rules. Perform self-audits to ensure compliance.
- Assists with transporting program participants to and from the Day Service programming.
- Plans coordinates and facilitates events to provide opportunities which are broadly based meet customer needs and serve diverse interests and abilities.
- Arranges all Chance event activities to include but not limited to logistics food beverages decorations music staff volunteers photography and admission.
- Provides input feedback and event ideas to create long-term sustainability.
- Researches and consults with customers to determine their interests suitability of events and attractiveness of offerings.
- Prepares and maintains registration database for all Chance events.
- Assesses customers fit within each event to ensure a cohesive group and mitigate potential conflict.
- Provides a safe memorable and exceptional customer service experience which exceeds consumer expectations and promotes continued participation.
- Responds promptly and competently in emergency response situations.
- Assists escorts and provides physical and other types of assistance needed by customers during events.
- A Bachelor Degree in a Human Services or Similar Field is preferred. Experience in Human Services or related field may be considered. A High School Diploma or equivalent with requested years of direct care and administration experience will be considered.
- Three years experience in direct care services and the administration of Adult Day Service programming caring for persons with disabilities preferred.
- Proficiency in Microsoft Suite Package to include Excel Outlook Word etc.
- Excellent communication skills written and oral strong organizational skills.
- Must be at least 21 years of age.
- Must possess a valid WI drivers license (where required) and ability to meet agency driving standards. Must provide proof of auto insurance (where required).
- Must pass a periodic WI Caregiver Background Check including Department of Justice Criminal Record Checks and/or that of previous states of residence.
