Job Summary
Full time 8:00AM-5:00PM Monday-Friday
The Accountable Care Supervisor provides day-to-day leadership and operational oversight for the Accountable Care team within Population Health. This position is responsible for coordinating staff activities monitoring productivity and performance supporting quality and risk-adjustment initiatives and ensuring timely completion of patient and practice-based workflows.
The Supervisor works closely with the Director of Population Health and other Population Health teams to support organizational goals related to quality preventive care value-based care and patient outcomes.
Education
- Bachelors degree in healthcare administration business public health nursing or related field preferred.
- Relevant healthcare experience may be considered in lieu of degree requirements asappropriate.
Experience
- Three years of healthcare experience preferred.
- Experience in physician practice operations population health quality improvement or accountable care preferred.
- Previoussupervisory or team-lead experience preferred.
