Were looking for a passionate organised and proactive Transition Support Officer to join our dedicated team and play a vital role in supporting students as they prepare for life beyond is a rewarding opportunity to work closely with young people with disabilities and additional needs helping them identify and secure meaningful next steps including accommodation education volunteering employment and community in Hereford but supporting learners across National Stars sites this role offers variety autonomy and the opportunity to make a genuine difference every the RoleAs a Transition Support Officer you will work as part of a multi-disciplinary team providing high-quality information advice and guidance to students families carers and external stakeholders throughout a learners will act as the main transition contact for a designated caseload of students creating personalised and aspirational transition plans that support positive post-college collaboratively with local authorities health professionals careers services families and internal colleagues youll ensure learners are equipped with the support opportunities and resources they need to achieve their ResponsibilitiesManage a caseload of students and provide tailored transition information advice and personalised transition plans that support learners from their first year through to their destination after as a key point of contact for families carers local authorities and external accommodation education employment volunteering and community opportunities for collaboratively with Learner Journey Coordinators Job Coaches Careers staff and wider multidisciplinary and maintain accurate records reports and case notes using college to annual reviews and support achievement of EHCP and maintain accessible information and guidance resources for in student reviews transition events focus groups and team quality assurance activities and preparation for Matrix Ofsted and CQC YouWere looking for someone who is passionate about achieving positive outcomes for young people and who can confidently build relationships with a wide range of of providing information advice and managing an individual working with external agencies such as Careers Services Social Services NHS teams or local organisation and time management written and verbal communication research and problem-solving IT skills and experience maintaining accurate to work independently and manage competing 2 English and Mathematics (or equivalent).Ability and willingness to travel as 3 Information Advice and Guidance qualification or within education training or supported learning working with young people with learning difficulties and/or of benefits and support available to disabled using databases and information management Join National StarMake a lasting impact on the lives of young people and their within a supportive and collaborative multidisciplinary opportunities for professional development and career a varied and meaningful role where every day is from flexible working options including consideration of part-time term-time only and job-share and InclusionNational Star is committed to safeguarding and promoting the welfare of children and young adults.
