Join our team at HMP Channings Wood as a Through the Gate Worker and help people prepare for life beyond prison. Youll support individuals approaching release to access treatment healthcare and community services helping them build positive futures sustain recovery and successfully reintegrate into their communities.
Experience in substance misuse criminal justice mental health social care or a related field can be beneficial but it isnt essential. If you have strong people skills can build positive relationships and are passionate about helping others well provide the training and support needed to help you succeed in the role.

Hours:37.5 per week
Contract:Permanent
Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles the salary and payments will be pro rata based on contracted hours
This is a rolling recruitment campaign and applications will be reviewed as they are received. The vacancy will close once a suitable candidate has been appointed so early applications are encouraged.
- Support individuals during the 12 weeks leading up to their release from prison.
- Develop personalised resettlement and recovery plans.
- Coordinate referrals to healthcare treatment and community services.
- Work closely with prison staff healthcare teams resettlement teams and external partner agencies.
- Advocate for individuals to access the support they need.
- Promote recovery resilience social reintegration and independence.
- Contribute to risk management and safeguarding processes where required.
- Enjoy helping and supporting others.
- Communicate effectively and build positive relationships.
- Are organised and able to manage competing priorities.
- Work well independently and as part of a team.
