Job Summary

Liquid Personnel is recruiting Social Worker for its clients Transition to Adulthood Team at Cornwall.

This is an excellent opportunity for a dedicated practitioner to support young people with care and support needs as they move from Childrens Services into Adult Social Care.

The successful candidate will work closely with young people their families Childrens Services Health Services and partner agencies to ensure a smooth and planned transition into adulthood. Using a strengths-based and person-centred approach you will undertake assessments coordinate support and help young people achieve positive outcomes whilst promoting independence wellbeing and choice.

This role requires a confident Social Worker with experience of transition work strong assessment skills and a sound understanding of the Care Act 2014 and Adult Social Care legislation.

What will your responsibilities be

  • Undertaking Care Act 2014 needs assessments with young people and their families to determine eligibility for Adult Social Care services.
  • Supporting young people through the transition from Childrens Services into Adult Social Care.
  • Working collaboratively with Childrens Services Health Services education providers carers and partner agencies to ensure seamless transition planning.
  • Developing and reviewing person-centred Care and Support Plans that promote independence wellbeing and positive outcomes.
  • Supporting young people and their families to understand available services support options and transition pathways.
  • Undertaking Mental Capacity Assessments and contributing to Best Interest decision-making in accordance with the Mental Capacity Act 2005.
  • Assessing and managing risk appropriately whilst promoting independence and positive risk-taking.
  • Undertaking safeguarding enquiries and responding to safeguarding concerns in accordance with Adult Safeguarding legislation and local procedures.
  • Coordinating services and support arrangements to ensure young people receive the right support at the right time.
  • Working closely with multidisciplinary teams to achieve joined-up planning and effective outcomes.
  • Preparing high-quality assessments reports reviews and support plans in accordance with statutory requirements.
  • Maintaining accurate timely and legally compliant case records and documentation.
  • Promoting strengths-based practice and supporting young people to maximise life opportunities as they move into adulthood.

Qualifications & Experience:

To be successful in this role you must have

  • Professional Qualification: MA BA BSc in Social Work Diploma in Social Work CQSW CSS or equivalent recognised Social Work qualification.
  • Registration: Must be registered with Social Work England.
  • Experience: Significant post-qualified experience within Childrens Services Adult Social Care or Transition Services.
  • Transition Experience: Experience supporting young people with complex needs through transition to adulthood is highly desirable.
  • Legislation Knowledge: Strong understanding of the Care Act 2014 Mental Capacity Act 2005 Adult Safeguarding legislation and associated statutory guidance.
  • Assessment Skills: Proven ability to undertake Care Act assessments support planning reviews and risk management interventions.
  • Safeguarding Experience: Experience undertaking safeguarding enquiries and managing safeguarding concerns.
  • Multi-Agency Working: Strong experience working collaboratively with Childrens Services Health Services education providers families carers and partner agencies.
  • Communication Skills: Excellent communication assessment report-writing analytical and decision-making skills.
  • Professional Practice: Commitment to strengths-based person-centred practice and supporting positive outcomes for young people and their families.
  • Organisational Skills: Ability to manage competing priorities and work effectively within a multidisciplinary environment.
