Social Worker Preparing for Adulthood

What will your responsibilities be

• Supporting transitions planning -working with young people (1425) to develop personcentred transition plans that promote independence wellbeing and longterm outcomes.
• Completing Care Act assessments -assessing eligible needs as young people move into adulthood creating strengthsbased support plans and coordinating appropriate adult services.
• Multiagency coordination -collaborating with education health SEND teams and families to ensure smooth transitions and consistent support across childrens and adults services.

Qualifications & Experience:

To be successful in this role you must have

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Degree level or equivalent in Social Work
