Social Worker Older People's Team

Liquid Personnel is recruiting Social Worker for its clients Older Peoples Team at Surrey.
This is an excellent opportunity to work with older adults and their families delivering strengths-based interventions that promote independence wellbeingand positive outcomes.

What will your responsibilities be

• Undertake Care Act 2014 assessments reviews and reassessments.
• Complete strengths-based person-centred assessments and support planning.
• Manage a caseload of adults with learning disabilities and autism.
• Coordinate care and support services to meet assessed needs and promote independence.
• Work collaboratively with multidisciplinary teams health partners carers and families.
• Advocate for service users rights choices and wellbeing.
• Undertake Mental Capacity Assessments and contribute to Best Interest decisions.
• Identify and respond to safeguarding concerns in line with statutory procedures.
• Assess and manage risk while promoting positive outcomes and independence.
• Maintain accurate timely and legally compliant case records and reports.

Qualifications & Experience:

To be successful in this role you must have

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Professional Qualification: MA BA BSc in Social Work Diploma in Social Work CQSW CSS or equivalent recognised Social Work qualification.
