Social Worker – Mental Health
Job Summary
Our client based in Swindon is seeking an experienced Social Worker to join their Mental Health Team. This is an excellent opportunity for a dedicated practitioner to work within a progressive and supportive service that is committed to helping adults with mental health needs achieve independence wellbeing and positive life outcomes.
The successful candidate will work within a multidisciplinary environment undertaking complex assessments support planning safeguarding interventions and risk management activity. You will play a key role in supporting individuals and their carers through challenging situations whilst promoting recovery strengths-based practice and person-centred approaches that place individuals at the heart of decision-making.
This role offers the opportunity to work collaboratively with a wide range of health and social care professionals contributing to high-quality mental health services that support people to live fulfilling and independent lives.
Key Responsibilities
Core duties include:
- Undertaking complex person-centred assessments including Care Act Assessments Mental Capacity Assessments and Section 117 Aftercare Assessments.
- Developing and implementing outcome-focused Care and Support Plans that promote recovery independence and wellbeing.
- Supporting individuals and carers through complex and sensitive situations ensuring appropriate interventions and support are in place.
- Undertaking Mental Capacity Assessments and leading Best Interest decision-making processes where required.
- Managing risk effectively through robust assessment planning and risk management interventions.
- Undertaking Section 42 Safeguarding Enquiries and responding appropriately to safeguarding concerns.
- Chairing and facilitating Best Interest meetings and other multi-agency meetings where appropriate.
- Working collaboratively with health services Community Mental Health Teams carers housing providers and partner agencies to achieve positive outcomes.
- Maintaining accurate timely and high-quality assessments case records reports and support plans.
- Applying strengths-based practice and relevant legislation to support individuals to make informed choices and maximise independence.
- Providing advice guidance and support to colleagues within the team and contributing to a positive learning culture.
Qualifications & Experience
- Professional Qualification: MA BA BSc in Social Work Diploma in Social Work CQSW or equivalent recognised Social Work qualification.
- Registration: Must be registered with Social Work England.
- Experience: Significant post-qualified experience within Adult Social Care or a related setting.
- Mental Health Experience: Experience working with adults with mental health needs is desirable.
- Legislation Knowledge: Strong understanding of the Care Act 2014 Mental Capacity Act 2005 Section 117 responsibilities Adult Safeguarding legislation and associated statutory guidance.
- Assessment Skills: Proven ability to complete complex assessments support planning safeguarding enquiries and risk assessments.
- Strengths-Based Practice: Experience applying person-centred and strengths-based approaches within Adult Social Care.
- Multi-Agency Working: Experience working collaboratively with health professionals and partner agencies to coordinate effective support.
- Communication Skills: Excellent communication report-writing analytical and decision-making skills.
