Social Worker Learning Disabilities

Liquid Personnel is recruiting Social Worker for its clients Learning Disabilities Team at Middlesbrough.

What will your responsibilities be

• Conduct strengths-based reviews of adults with learning disabilities living in Independent Supported Living (ISL) schemes ensuring support arrangements promote independence wellbeing choice and positive outcomes.
• Complete person-centred assessments and reviews working closely with individuals families providers and the Accommodation Lead to evaluate support needs identify risks and ensure placements remain appropriate.
• Maintain accurate case records and review documentation undertake home visits as required and contribute to care planning and decision-making in line with Care Act duties safeguarding procedures and local authority policies.

Qualifications & Experience:

To be successful in this role you must have

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Degree level or equivalent in Social Work
