Social Worker Hospital Team

Liquid Personnel is recruiting anAdult Social Worker to join its clients Hospitalteam based in Manchester.

What will your responsibilities be

• Managing hospital discharges - completing timely Care Act assessments coordinating safe discharge plans and ensuring appropriate community support is in place.
• Liaising with reablement and MDTs - working closely with therapy teams ward staff and external providers to progress discharge pathways and reduce delays.
• Undertaking hospitalbased social work tasks - addressing safeguarding concerns assessing risk updating support plans and supporting complex cases within a fastpaced acute setting.

Qualifications and Experience:

To be successful in this role you must have;

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Degree level or equivalent in Social Work
