Social Worker Hospital Discharge

What will your responsibilities be

• Manage a hospital-based social work caseload.
• Complete assessments and support planning to facilitate safe discharges.
• Work closely with NHS professionals patients families and partner agencies.
• Coordinate care packages and community support to prevent delayed discharges.
• Undertake safeguarding responsibilities and risk assessments where required.
• Maintain accurate and timely case records.
• Provide cross-site cover across multiple hospital locations as required.
• Participate in weekend working on a rota basis.

Qualifications & Experience:

To be successful in this role you must have

• Qualified Social Worker (BA MA DipSW CQSW or equivalent).
• Registered withSocial Work England.
• Previoushospital social work experienceis essential.
• Experience supportinghospital discharge planning.
• Strong assessment communication and multi-agency working skills.
• Ability to work flexibly across hospital sites and weekends.

Compliance Requirements:

• Minimum of3 years references.
• Full employment history with any gaps explained.
• Relevant qualifications in line with the role requirements.
