Your role and responsibilities
As a Senior Independence and Wellbeing Practitioner you will play a key role in helping adults achieve the best possible outcomes maintain their independence and improve their wellbeing. Working within a supportive multidisciplinary team you will undertake strengths-based assessments and reviews ensuring people receive the right support at the right time.
In this rewarding role you will:
- Complete Care Act assessments and reviews to identify eligible care and support needs.
- Develop arrange and review personalised care and support packages.
- Apply the principles of the Mental Capacity Act 2005 to support informed decision-making.
- Respond to safeguarding concerns in line with local policies and procedures.
- Complete NHS Continuing Healthcare checklists and contribute to the wider assessment process.
- Undertake telephone assessments home visits and face-to-face conversations with people and their families.
- Work closely with colleagues across health social care and voluntary sector services.
- Attend and contribute to professional and multi-agency meetings.
- Carry out lone working within the community where required.
This is primarily a community-based role involving regular travel home visits and face-to-face assessments. Hybrid working is supported with administrative work undertaken from Riverside or from home where appropriate.
You will receive a comprehensive induction including shadowing opportunities and access to training and professional development.

The team
This is an exciting time to join our Mental Health Service within Adult Social Care and play a key role in improving outcomes for people across Suffolk.
We have invested in expanding our Mental Health Learning Disabilities and Autism services creating additional opportunities for qualified Social Workers Senior Independence and Wellbeing Practitioners and Occupational Therapists. This investment reflects our commitment to delivering high-quality person-centred support that helps people live independently and achieve their goals.
We have also brought operational delivery and service development teams together creating a more integrated approach that drives innovation supports best practice and helps shape new pathways and ways of working across the service.
You will work collaboratively with a wide range of partners including Norfolk and Suffolk NHS Foundation Trust Children and Young Peoples Services care providers and community organisations ensuring people receive coordinated and effective support.
