This role will deliver across one or more programmes that make up the NHS Healthier You service. These programmes include the Diabetes Prevention Programme (DPP) Type 2 Diabetes Pathway to Remission Programme (T2DR) Behavioural Support for Obesity Prescribing Programme (BSOP)and Long-Term Conditions Prevention Pilot (LTCP).
As a Self-Employed Health & Wellbeing Coach youwillbe responsible forthe engagement and delivery of sessions inbothgroup and/or one-to-one settings with participants.
Sessions are delivered face-to-face across Devon therefore flexibility and a willingness to travel are essential. Applicants must also have access to their own vehicle and hold a full UK driving licence.
Expected availability for programme delivery is between12 and 20 hours per week.In returnyou will receive50 per 90 Minute session.
Just some of yourday-to-dayresponsibilities will include:
Deliver engaging NHS Healthier You Programme sessions to participants in a group and/or one-to-one settingin accordance withour curriculum resources and qualitystandards.
Motivate support and empowerparticipants to make sustainable lifestyle and behaviour changes using evidence-based coaching techniques toimprove theiroverallhealth and wellbeing.
Adapt session delivery to meet the diverse needs abilities and learning styles of participants and create a positive and inclusive environment to encourage participation.
Monitor participant progress conduct regular revies and celebrate achievements whileidentifyingadditionalsupport needs.
Signpost participants toappropriatelocalhealth wellbeing and lifestyle support services whererequired.
Participate in training professional development and quality improvement activities to ensure the highest standards of service delivery.
Contribute ideas and feedback to support continuous improvement of programme delivery and participant outcomes.
To be successful in this role we are lookingforsomeone with:
Essential Qualifications:
MinimumLevel 3qualificationinExercise ReferralHealth Improvement/Promotion/Public HealthNutrition&Weight ManagementCoaching&Behaviour Change orEducation & Training.
Evidence of training qualification or assessed competence inbehaviour change delivery aligned with recognised frameworks (e.g. COMB Behaviour Change Wheel motivationalinterviewingor health coaching approaches).
Fully First Aid trained with a valid certificate.
EssentialSkills & Experience:
Experiencefacilitatinggroups and supporting individuals to achieve positive health lifestyle or behaviour changes.
Strong communicationand relationship-building skills with a person-centred and non-judgemental approach.
Ability to engage and motivate people from diverse backgrounds and with varying levels of confidence and health literacy.
Demonstrable IT skills including the ability to confidently use digital platforms and systemsto deliver services record data and meet reporting requirements.
Willingness toparticipatein induction supervisionobservedpractice competency assessment and ongoing CPD including all mandatory core training.
Flexibility to work evenings and weekends as required and the ability to travel throughout the designated region using your own vehicle.
Desirable Criteria:
Experience delivering behaviour change programmes aligned withNHScommissionedor public health services.
Experience working with individuals and/or groups withadditionalneeds long-term health conditions obesity or pre-diabetes.
Fluency in multiple languages.
Experience supporting service users via digital or remote delivery platforms.
Qualification in group-baseddelivery.
Level 3 Qualification in British Sign Language.
