We have a new opportunity to join our team in Norwich based at Bowthorpe Community Hospital.
- Take referrals into our service by phone and Liquid Logic (our social care system)
- Set up visits to service users for support workers using an electronic scheduling software (full training will be given)
- Take queries from the support workers while they are working in the community
As a Reablement Liaison Officer you will be the first point of contact and an information service for GPs community health services social services service users and their families. You will also take referrals for Norfolk First Support.
Norfolk First Support delivers services to adults across Norfolk in their own homes for up to 6 weeks assisting them to regain skills they may have lost following an illness or hospital admission enabling them to regain their independence as much as possible while also reviewing any ongoing support needed.
You need to possess excellent ICT skills and be able to work to tight deadlines often making decisions independently whilst delivering excellent customer service. You will also have the ability to assess risk and take appropriate action working with our multidisciplinary team to support people to return to their homes.
The role is hybrid working both from home and a locality rota is on a two-week or four-week rota basis which includes working morning and evening shifts bank holidays and may also include alternative weekends but this is planned in advance to help us deliver our service whilst enabling you a healthy work-life hours are paid at a higher rate of time and a third.
