A fantastic new opportunity has arisen to become aProject Managerleading the delivery and service development of CGL Nottinghamshires RSDAT Assertive Outreach Team. You will form part of our Severe Multiple Disadvantages management team sharing our passion for making every adult matter.

*Please note:Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles the salary and payments will be pro rata based on contracted hours.
Responsibilities
You will focus on engaging with people throughout Nottinghamshire who have a history of rough sleeping and substance use. You will have some experience of engaging with individuals with severe multiple disadvantages and demonstrate a determined approach to finding solutions. Comprehensive knowledge of one or more of the following is required:
Homelessness
Mental health
Criminal Justice System
Drugs and/or alcohol use
About the Role:
Lead on the development of the outreach provision building a holistic service that meets the needs of those who experience severe multiple disadvantages
Embed elastic tolerance and trauma informed practice throughout the team ensuring that staff are well trained and well supported including engaging in supervision and appraisal process
Map and co-ordinate the outreach provision across the county ensuring resources are equitably allocated
Participate and lead in multi-agency/disciplinary forums identifying gaps and pathways for improvement
Ensure that contractual output targets are achieved and produce reports in a timely manner in order that the service meets contractual administrative and financial requirements.
Provide line management supervision and leadership to achieve performance outcomes in line with service level and contractual arrangements.
In conjunction with the Service Manager and Senior Management Team identify areas of service development.
Oversee the monitoring and evaluation of the service offered to clients and actively engage involve and consult with service users and family and carers regarding on-going service development.
About you:
Considerable experience managing a team within the health & social care sector with a proven track history of delivering quality services
A demonstrable understanding of performance management frameworks effective governance and quality management.
Experience in the implementation and development of new services evidencing a broad understanding of pathway integration
Knowledge and understanding of the possible recovery pathways available across whole treatment and recovery systems including community residential and Prison provision.
Experience of staff supervision and appraisal.
Well-developed leadership skills.
Experience of data collection systems information technology.
Comprehensive knowledge of severe multiple disadvantages including barriers to support
Possess or be working towards a recognised qualification in health social care or management.
